The original requisition set up by the dispatching enterprise for establishing representative institution.

3、董事會(huì)指定的委托代理人的委托書(shū)，（全體董事簽名，加蓋公章）；

A power of attorney of the clients appointed by the directorate ( signed by all the directors and seal).

4、外國(guó)公司的合法開(kāi)業(yè)證明（合法開(kāi)業(yè)證明為企業(yè)所在國(guó)家注冊(cè)機(jī)關(guān)核發(fā)的注冊(cè)原件，如提交復(fù)印件需經(jīng)所在國(guó)家公證處公證或我國(guó)駐當(dāng)?shù)厥诡I(lǐng)館認(rèn)證）

7、首席代表的個(gè)人資料包括： 工作簡(jiǎn)歷（本人簽名），身份證，回鄉(xiāng)證，護(hù)照原件及復(fù)印件，證件照10張；

The personal documents of the Chief Representative: resume of working, ID card, reentry permit, the original and copy of the passport, ten pieces of photos for credentials.

8、該代表機(jī)構(gòu)住所的租賃合同（經(jīng)房管部門備案），產(chǎn)權(quán)證明；

Representative offices rents contract (must be taken to the housing administration bureau to register a file), and the house-owner property certificate;

注：（1）第2項(xiàng)總公司為香港公司的，開(kāi)業(yè)注冊(cè)證明需經(jīng)香港具中國(guó)委托公證人的律師樓公證，總公司為外國(guó)公司的，注冊(cè)登記證明需經(jīng)中國(guó)駐當(dāng)?shù)卮笫桂^蓋章；

   （2）以上資料若為外文，須附中文譯文

Note: (1)For item four, if the head office is in Hong Kong, the certificate of register needed to be notarized by the Hong Kong lawyer office with the Chinese delegated notary public; if the head office is abroad, the certificate of register needed to be sealed by the local Chinese embassy;
